CharterCARE Home Health Services works to get our patients back on their feet in the safety and comfort of their own home, under the supervision of their physician, visiting nurses, therapists, and aides.

CharterCARE Home Health Services is a hospital-based visiting nurse service licensed by the Department of Health, accredited by the Joint Commission. Our patient satisfaction surveys tell us that our patients are very happy with our services.

Our record for quality is confirmed by national accrediting agencies. Following a four day survey in June 2017, CharterCARE Home Health Services earned The Joint Commission’s Gold Seal of Approval for accreditation by demonstrating compliance with The Joint Commission’s national standards for health care quality and safety. The accreditation award recognizes our Home Health Services' dedication to continuous compliance with The Joint Commission’s state-of-the-art standards.

Initially established as Roger Williams Medical Center Home Care in 1987, we are a fully licensed, Medicare and Medicaid-certified, home health agency. We believe it is equally important for us to understand, reassure and respect the families and significant others of our patients. Our Mission

We seek to attain excellence in the home health care services. We foster communication and collaboration between all employees and specialties to ensure the optimum quality care and superlative service. It is our goal to be the home health care provider and employer of choice in Rhode Island and bordering Massachusetts.

Our Services

We have established a patient-centered, primary care approach to the delivery of in-home care. The care we offer is delivered by skilled nurses, CNAs, certified rehabilitation specialists, medical social workers, and registered dieticians. We provide our services to clients in Rhode Island and some bordering towns in Massachusetts.

Our Medical Director

John A. Stoukides, MD, ScD., has been practicing medicine for 32 years and is board-certified in Internal medicine, Hospice and Palliative Medicine, Quality Assurance, and Utilization Review. He is Vice Chairman of the Department of Medicine and Chief of the Division of Geriatrics and Palliative Medicine at Roger Williams Medical Center and also serves as Medical Director of Utilization Management for CharterCARE Health Partners. He has been overseeing the COVID vaccination program at CharterCARE hospitals where they have successfully inoculated more than half of the system’s approximately 3,000 employees. He received his pharmacy degree from URI and medical degree from Tufts University School of Medicine along with an Honorary doctor of science in Geriatrics from URI in 2005. He serves as a Clinical Professor of Pharmacy at URI, Assistant Professor of Medicine at Boston University School of Medicine and Warren Alpert Medical School at Brown University, and is Associate Professor of Nursing at URI. Dr. Stoukides has a private practice in East Providence.

Personal Emergency Response System

Our Personal Emergency Response System (Lifeline program) is designed to reduce the risk of living alone. In the event of a fall or emergency, help is available at the push of a button. The personal help button connects you to a trained Personal Response Associate who can send help quickly to your home, 24 hours a day, 7 days a week.
